Job summary

Catholic Guardian Services in the Bronx, NY is seeking a compassionate Case Planner for the Enhanced Family Foster Care program. You will provide case management, connect families with resources, and promote safe, stable, and permanent homes.

Requirements include a Bachelor's degree (BA/BS/BSW), 1–2 years in child welfare, strong communication and organizational skills, and ability to travel throughout NYC; MSW is preferred and a driver's license is a plus.

Qualifications

• Bachelor's degree (BA/BS/BSW).
• 1–2 years of child welfare or related experience.
• Experience working with adolescents and families.
• Strong communication, organizational, and problem-solving skills.
• Ability to travel throughout NYC and work occasional evenings or weekends.
• Preferred: MSW and valid driver's license.

Responsibilities

• Provide case management and ongoing support to children, birth parents, and foster families.
• Develop and monitor individualized service plans to address family needs.
• Conduct home visits, assessments, and regular family meetings.
• Connect families with community resources and supportive services.
• Collaborate with foster parents, service providers, and interdisciplinary teams.
• Maintain accurate case documentation and support permanency planning.
