Qualifications

  • 1+ year of case management, foster care, child welfare, family services, or related human services experience is preferred.
  • Knowledge of child welfare, family engagement, permanency planning, trauma-informed care, safety assessment, and strength-based case management practices is preferred.
  • Proficiency with electronic case documentation, progress notes, case plans, Microsoft Office, and standard case management systems is required.
  • Strong communication, counseling, group facilitation, relationship-building, organization, documentation, collaboration, and ability to engage children and families are required, with bilingual English and Spanish skills preferred.

Responsibilities

  • Manage assigned foster care cases through ongoing assessment, family contact, service coordination, and individualized case planning.
  • Build supportive relationships with children, parents, and foster families while providing counseling, education, and guidance toward established goals.
  • Conduct required home and community visits to evaluate safety, placement stability, family needs, and progress.
  • Coordinate family and sibling visits and collaborate with internal and external partners to support reunification and other permanency objectives.
  • Maintain timely case records, progress notes, assessments, court-related reports, and other required child welfare documentation.
  • Connect families with appropriate community resources and collaborate with schools, healthcare providers, behavioral health professionals, courts, and other service partners.

Compensation: $28/hour

Location: Bronx, NY

Schedule: Monday to Friday (On-Site)

TAG MedStaffing is hiring a Foster Care Case Planner in Bronx, NY for our client, supporting foster care case management, family engagement, permanency planning, and child welfare services. The Foster Care Case Planner will work with children, parents, and foster families through office-based and community-based services while coordinating resources and monitoring safety and progress.
