Job summary

Qualifications

• Bachelor's degree in a human services field or nursing.
• Valid driver's license.
• Two years of experience with case management or related discipline.
• Ability to communicate both verbally and in writing.

Responsibilities

• Receives and responds to calls from referral sources to identify client needs and consult with Clinical Management regarding capacity to meet them.
• Completes viable referrals by pre-screening, preparing pre-screening reports, case proposals and related pre-admission paperwork.
• Oversees field staff assigned to assist with the pre-screening process.
• Maintains relationships with referral sources and payer contacts, tracking satisfaction and conversion metrics.
