Presbyterian Children's Homes and Services in St. Louis, MO seeks a Foster Care Case Manager to support safety and services for children and families in foster care.

Responsibilities include coordinating services, documenting progress, and facilitating Family Support Team meetings. Qualifications

• Bachelor’s degree in social work, psychology, counseling, education, criminal justice, or related human services field; Master’s degree preferred.
• Minimum of two years of relevant case management experience.
• Must have valid driver’s license and reliable transportation.
• Ability to visit more than one family per day and travel as needed.

Responsibilities

• Maintain a caseload of 12 to 15 children per program.
• Provide full case management services through assignment to case closure or transfer.
• Document activities and data entry in FACES and CSWIS.
• Maintain monthly face-to-face contact with each child and family; regular phone contacts.
• Attend court hearings and coordinate Family Support Team meetings.
