Centerstone seeks a Foster Care Case Manager to safeguard the welfare of foster children and coordinate services across families, schools and community providers. The role requires in-home visits, documentation in Centerstone EMR, and active liaison with DCS and foster families.

Mileage is reimbursed, and pay ranges from $16.50 to $23.10 per hour with full-time hours. Qualified candidates will have a bachelor’s degree in a behavioral health-related field and at least one year of relevant

• Bachelor’s degree required; degree in psychology, social work, criminal justice or related behavioral health field preferred.
• Minimum of 1 year experience with children, youth and families required.

Responsabilidades

• Provides safety, permanency and well-being for foster care clients and acts as primary liaison between Centerstone, DCS, foster families, biological families, schools and services.
• Conducts in-home visits and links clients/families to community resources; ensures follow-up on EPSDT and Dental requirements.
• Completes MAR in foster home and meets all EMR and DCS documentation requirements within specified timelines.
• Coordinates case management services with school personnel; arranges and supervises visits; assists foster parents with transportation as needed.
