Job summary

Arrow Child & Family Ministries is seeking a Case Manager to oversee treatment and coordination of services for foster/adopt children and their families. You will facilitate placements, develop treatment plans, and maintain meticulous records in Arrow systems.

Responsibilities include crisis intervention, collaboration with multidisciplinary teams, and ensuring compliance with licensing standards. A Bachelor's in social work or related field is required, with 1 year of relevant experience; a

Qualifications

• Bachelor’s degree in Social Work or related field is required.
• Minimum 1 year of experience working with children and families.

Responsibilities

• Facilitate placement of children in foster/adoptive homes and ensure intake paperwork is signed and entered into the electronic case management system within policy timeframes.
• Participate in development of assessment, treatment planning, service delivery, discharge, and aftercare as determined by the child’s service package.
• Assist and monitor families in licensing/regulatory compliance to maintain service package requirements and credentialing.
• Provide crisis intervention as needed to prevent escalation of high-risk behavior and ensure safety.
• Maintain accurate case notes, billing documentation, treatment plans, progress notes and other documentation.
• Coordinate care with medical consenter and participate in STAR Health Coordination applicable to TX.
