The position requires a BA in a related field and at least one year of relevant experience in protective services or case management, with strong teamwork and resource mobilization skills.

Qualifications

• Bachelor's degree is required in social work, counseling, psychology, education, sociology, or criminal justice.
• Minimum of one year of professional employment delivering protective social services, family preservation or targeted case management in a public or private agency.
• Demonstrated experience as a positive contributor in a close team and community relationship, and with community resource mobilization.

Responsibilities

• Conduct regular in-home visits to support children and families.
• Coordinate resources and services across medical, educational, emotional, and social domains.
• Maintain thorough documentation and track outcomes to support permanency goals.
• Coordinate transportation, legal support, and court appearances as needed.

As a case manager, you will make timely, independent decisions to support children and families with medical, educational, emotional, and social services. You will coordinate critical resources, ensure 24/7 emergency availability, lead family meetings, and guide families toward long-term permanency goals. This role involves regular in-home visits, close collaboration with caregivers, courts, and community partners, and a strong commitment to documentation and outcome tracking. Transportation, legal support, and attending court are also essential duties. Ultimately, you help ensure each child's safety, stability, and long-term success.
