Job summary

Families United Network is seeking a Case Manager to provide sensitive, proactive support to children and families. The role emphasizes planning to prevent problems, meeting policy requirements, and collaborating with agencies to promote safe, supportive environments.

Responsibilities include coordinating transportation, planning visits, maintaining accurate records, and ensuring timely reports while attending required trainings and court appearances.

Qualifications

  • Bachelor's degree preferred in social work or related field.
  • Two years' experience working with children and/or families preferred.
  • Eligibility under PA legal and regulatory requirements.

  • Provide sensitive, resourceful case management and active support to children and families.
  • Attend required meetings, court hearings, trainings, and agency functions.
  • Manage time, complete reports on time, and ensure documentation meets policies.
  • Assist and support resource families and children to develop safe, welcoming environments.
  • Arrange transportation and plan visits, returns home, and other plans with families and referring agencies.
  • Maintain complete, clear, organized records in compliance with regulations.

The Case Manager is responsible for being sensitive to problems and solving them in a resourceful way, for intelligently planning to prevent problems and increase opportunities, and for going beyond mere compliance and cooperation to active support, encouragement, and teamwork.

  • Report to the Program Office Supervisor.
  • Learn, understand, accept, and implement agency policy, applicable laws, and regulations.
  • Attend required meetings, court hearings, conferences, trainings, and other agency functions, as required.
  • Manage work time appropriately, be punctual for appointments, and complete assignments and reports on time.
  • Ensure that all documentation is written and reports are completed as required.
  • Assist and support resource families and children in developing relationships that will make possible and promote a safe, welcoming environment.
  • Ensure that transportation is available as needed, or transport children and families when appropriate.
  • Plan with children, resource families, birth families, and referring agencies regarding visits, returns home, and other plans.
  • Maintain complete, clear, comprehensive, neat, and organized child and family records in compliance with policies and regulations.
  • Participate in the process of accepting children referred for care in a manner that ensures that the needs, rights, and welfare of children and families are the priority.
  • Attend meetings and trainings for ongoing staff development.
  • Make plans, arrangements, and collateral contacts, or provide direct help through the use of available resources to ensure that problems are solved and children receive medical care, dental care, education, religious opportunities, supervision, clothing, opportunities for contact with referring agencies, family visits, recreation, and social opportunities in compliance with all regulation and policy requirements.
  • Perform additional duties as assigned.
Requirements
  • Bachelor's degree preferred, preferably in a field related to social work
  • Two years' experience working with children and/or families preferred
  • Eligibility under current PA legal and regulatory requirements
  • Valid PA Driver's License
  • PA Child Abuse Clearance, PA State Police Clearance, PA FBI Clearance
  • Sensitivity to service population’s cultural and socioeconomic characteristics
Physical Requirements
  • Ability to sit for extended periods while working at a computer or desk.
  • Ability to stand, walk, bend, and reach occasionally throughout the workday.
  • Ability to lift and carry up to 10-15 pounds (such as files, small office equipment, or supplies).
  • Manual dexterity sufficient to operate a computer keyboard, mouse, telephone, and other office equipment.
  • Ability to read screens, documents, and reports for prolonged periods.
  • Ability to communicate clearly in person, over the phone, and via electronic means.
  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
