Job summary

Sevita's Ohio Mentor program seeks a Foster Care Case Manager to coordinate services for youth in care and transition planning. You will ensure quality services, monitor compliance, and build strong relationships with families and external case managers.

Qualifications include a Bachelor’s in human services or related field, at least 1 year working with at-risk youth, licensure/training as required, and a valid driver’s license with reliable transportation.

Qualifications

• Bachelor’s degree in human services or related field is required.
• 1 year of work-related experience with at-risk youth.
• Equivalent combination of education and experience accepted.
• Valid driver’s license, vehicle registration, and auto insurance required.
• State-required licensure/training such as CPR/First Aid/Behavioral Intervention Techniques.
• QMRP/QIDDP as required by state.

Responsibilities

• Coordinate development, review and implementation of services, case management, behavior intervention plans, and treatments.
• Oversee services, monitor quality of care, ensure compliance with contracts and regulations.
• Assist in developing quality services and engaging activities for individuals served.
• Build and maintain relationships with families and external case managers.
