Job summary

SynergenX Health is seeking a Mid-Level Provider (NP/PA) to join our Fort Worth area clinics. You will work under a physician medical director and center director to coordinate care for hormone replacement and targeted wellness services.

The position is full-time with training provided, and requires an active Texas NP/PA license and current DEA license. You will float between Arlington, Mansfield, Fort Worth, Southlake and Alliance locations to ensure patient access.

Qualifications

• Active Texas NP or PA license in good standing.
• Current DEA license is a requirement.
• Willing to float between Arlington, Mansfield, Fort Worth, Southlake and Alliance locations.

Responsibilities

• Coordinate care of patients receiving hormone replacement and targeted wellness, in conjunction with the physician medical director and Center Director.
• Work across multiple clinic locations as needed to support patient access and continuity of care.
