Job summary

REM Community Services, a part of the Sevita family, seeks a Registered Nurse to join a Forensic Assertive Community Team (FACT) serving clients with serious mental illness and possible co-occurring conditions. You will conduct health/medication assessments, crisis planning, counseling, and case management.

The position emphasizes nursing care, medication monitoring, team collaboration, and on‑call responsibilities within a community setting.

Qualifications

• Associate arts degree in nursing or higher.
• Bachelor's degree in nursing with equivalent supervised experience.
• Graduate degree in nursing with post BA hours.

Responsibilities

• Work in partnership with the client in their recovery process.
• Provide health/medication assessment, crisis assessment, planning, counseling and case management.
• Deliver nursing care and administer medications per established practices.
• Coordinate with other health professionals and outside providers.
• Complete nursing documentation as required.
