Job summary

Centurion Health is seeking a PRN/Per Diem Residential Counselor in California to provide community-based services and monitoring for FACT clients, including judicially committed individuals. Qualifications

• High school diploma or GED required.
• At least 1 year of mental health experience required.
• Community-based mental health experience preferred.
• Must pass background investigation.
• Active CPR certification preferred.
• Valid California driver's license with good driving record; insurability requirements apply.

Responsibilities

• Provides socialization and recreational services to support recovery.
• Conducts structured skills-based groups to address coping skills and daily living.
• Supports client's transition to community housing and safety planning.
• Maintains encounter notes detailing services and client responses.
• Coordinates with clinical staff to address concerns and referrals.
• Collaborates with multidisciplinary team for wellness and recovery planning.
