You will work under the supervision of the Program Director to coordinate benefits, housing applications, and links to community health services.

Qualificações

• Bachelor’s degree required with relevant field
• Experience with mental health, crisis intervention, and trauma-triggered behavior
• Excellent time management, flexibility, and teamwork
• Willingness to CPR/First Aid/AED/CPI/Naloxone

Responsabilidades

• Provide case management and benefits assistance, housing referrals in apartments.
• Assist in developing Support Plans and document in electronic records.
• Motivate clients to develop skills for independent housing and utilize natural supports.
• Ensure timely documentation and regulatory compliance.
• Assist with medication monitoring and review medication cart.
• Co-facilitate groups on wellness, housing, employment, and education.
• Prepare program for audits and assist with admissions/discharges.
• Provide crisis intervention when needed.
• Coordinate with other departments and outside agencies.
