Job summary

Qualifications

• Bachelor’s degree required in a relevant field.
• 1–3 years related experience in mental health or social services.

Responsibilities

• Perform forensic case management assessments and discharge planning.
• Provide supportive counseling and intervention; connect clients with community services.
• Maintain clinical charts and progress notes; monitor treatment plans.
• Coordinate with therapists, staff and doctors to discuss client progress.
• Assist with intake interviews and pre-screening; participate in trainings and case reviews.
• Ensure compliance with program policies and procedures.
