Job summary

LifeStream Behavioral Center in Leesburg, FL is seeking a Forensic Case Manager to join a multidisciplinary team. You will support individuals returning from forensic hospital settings or diversion programs, coordinating with families, courts, DCF, and community agencies to build progress toward independent living.

You will document case activities accurately, develop person-centered plans that meet court requirements, monitor progress, and connect clients to needed services beyond the program.

Qualifications

  • Bachelor’s Degree in Human Services or related field required.
  • Must receive certification as targeted case manager within six (6) months of hire
  • A minimum of one (1) year experience in behavioral healthcare and/or one (1) year experience in criminal justice
  • Must be Over the age of 21
  • Minimum of 5-years driving experience
  • Possess a Valid FL Driver’s license (within 30 days of hire if currently do not possess)
  • Possess a continuously valid US Driver’s license for the past three years, from date of issue required
  • Safe Driving record required

Responsibilities

Job description
