Job summary

Hill Country Mental Health and Developmental Disabilities Centers is seeking a Care Coordinator to assist with daily curriculum, case-management, and for JBCR participants. You will document progress, oversee discharge planning, and coordinate with jail, court, and administrative staff.

You will also lead forensic instructional programming, conduct group and art therapy activities, and ensure confidential clinical documentation is maintained for the court.

Qualifications

• Bachelor's degree in social, behavioral, or human services required.
• Credentialing as QMHP-C'S preferred/possible within credentialing process.
• Strong ethics and confidentiality in handling sensitive information.

Responsibilities

• Assist with daily curriculum and case-management services for JBCR program.
• Document individuals' progress and monitor competency restoration trajectory.
• Coordinate discharge planning and transition services with court and jail staff.
• Provide forensic instructional programming and group/art therapy activities.
• Maintain accurate clinical documentation shared with the court.
• Collaborate with jail, court, and administrative staff.
• Attend team meetings and work collaboratively with clinical team.
