Job summary

Proactive MD seeks an experienced Family Nurse Practitioner to deliver comprehensive primary care at our Florence, SC onsite health center. Hours are Monday–Friday 8:00am–5:00pm (40 hours/week), with no weekends. Our model emphasizes patient-centered care with time to listen and educate.

We offer broad-scope primary care with a patient-first approach, focusing on long-term health outcomes, preventive care, and collaborative team-based care in a non-fee-for-service environment.

Qualifications

• Master's degree in Nursing (MSN).
• FNP certification.
• Minimum of 3 years of experience in family practice or primary care.
• Current RN license.
• Current NP license in state of practice.
• Current BLS certification (AHA or ARC).
• Prescriptive authority under state and federal laws.
• Experience with EMR; athenahealth used by Proactive MD.

Responsibilities

• Provide comprehensive primary care to adult and pediatric patients in an employer-sponsored health center.
• Conduct preventive, acute, and chronic care visits.
• Perform physical examinations, health assessments, and medical histories.
• Diagnose and treat common conditions and develop individualized plans.
• Provide health education and wellness counseling.
• Order and interpret labs and diagnostics; coordinate referrals.
