Job summary

Vantage-At-Hampden is seeking a Speech-Language Pathologist for a full-time floating position between our Hampden and Sixteen Acres facilities in Springfield, MA. The role requires M.A./M.S. in Speech-Language Pathology and CCC or CFY eligibility, with PT/PRN opportunities at both sites.

You will assess residents, develop care plans, document progress and ensure HIPAA-compliant, high-quality therapy while collaborating with rehab staff to meet facility goals.

Qualifications

• M.A. or M.S. in Speech-Language Pathology from an accredited program.
• CCC or CFY eligibility and licensure in process or obtained.
• Ability to plan, implement and direct speech therapy programs.

Responsibilities

• Plan, develop, organize and direct speech therapy services per regulations.
• Assess residents appropriate to the age of the patient.
• Maintain accurate documentation and department records.
• Communicate effectively regarding ST programs and workflows.
• Discharge planning and formulation of resident care plans.
