Qualifications

• Licensed or eligible for licensure in WI
• Immunization and non-vaccine injectable certification may be required
• Bachelor’s or Doctorate in pharmacy; NABP FPGEC acceptable if applicable
• Strong computer skills and ability to document in EHR/dispensing systems
• Excellent communication, organization, and presentation/training abilities
• Life support training may be required depending on department

Responsibilities

• Oversees pharmaceutical care and evaluates biomedical literature to design medication therapy regimens
• Counsels patients and caregivers on medication therapy, adverse effects, compliance, and proper administration
• Designs and monitors patient-specific pharmacotherapeutic regimens and implements recommendations
• Reviews medication orders and prescriptions for accuracy
• Participates in medical emergency management when applicable
• Supports medication safety and quality improvement initiatives
• Manages formulary adherence and assists in acquiring non-formulary or investigational medications
• Ensures compliance with organizational policies and state/federal regulations
• Prepares, dispenses, and oversees storage and handling of medications
• Serves as a preceptor and mentors pharmacy teammates, residents, and students
• Directs and checks the accuracy of pharmacy support staff work
