Job summary

Project HOME in Philadelphia seeks a Floating Residential Services Coordinator to support residents and staff across multiple sites. You will help orient residents, coordinate resources, and promote wellness through a person-centered recovery model, assisting with goal setting, housing retention, and skill development.

This role requires strong communication, collaboration with housing and health teams, and on-call availability.

Qualifications

• High School Diploma or GED with 4 years of experience working with the population; or an Associate's Degree with 2 years of experience; or a Bachelor's Degree.
• Experience working with individuals who have experienced homelessness and/or behavioral health challenges.
• Availability for evening, weekend, and on-call duties.

Responsibilities

• Assist residents in transitioning from homelessness by providing initial support, orientation, and engagement to establish trust and well-being, while coordinating resources tailored to their individual needs and barriers.
• Collaborate with residents to design and implement recovery plans with short- and long-term goals, facilitate crisis intervention when needed, and advocate in housing, benefits, employment, education, and medical care.
• Plan and facilitate community-building activities, groups, and house meetings to enhance residents' self-esteem and life skills, while encouraging participation in site-based and Project HOME resources and programming.
• Maintain accurate and timely resident records, including case notes and assessments, attend necessary meetings and training, alternate on-call responsibilities, and serve as the point of contact in the absence of program management.

About the Role

As the Floating Residential Services Coordinator, you will provide essential support during staff vacancies or special projects, engaging with residents and staff to understand site operations and maintain daily programming. This role focuses on promoting wellness and recovery in an LGBTQ-affirming environment through a person-centered recovery model, assisting residents in achieving their goals, retaining housing, developing skills, and enhancing self-sufficiency.

• Location: Various Sites-Philadelphia, PA
• Schedule:Full -Time -Mon-Fri (8AM-4PM) | (9AM-5PM) |10AM-6PM
• Compensation: $23.09 per hour

What You Will Do

Site Specific Duties and Responsibilities ASAPPLICABLE

• Assist andsupport meal preparation with residents - at sites with kitchens or mealservices.
• Medicationmonitoring and support the organization of the medication room - atsites with medication-monitoring duties.

What You Will Bring

• High SchoolDiploma or GED with 4 years of experience working with the population; or an Associate'sDegree with 2 years of experience; or a Bachelor's Degree.
• Experience working with individuals who haveexperienced homelessness and/or behavioral health challenges.
• Availabilityfor evening, weekend, and on-call duties.

Core Skills:

• Good verbal andwritten communication, organizational and interpersonal skills
• Strong computerskills and ability to learn computerized case management system.

Physical requirements:

• Mobile work thatinvolves traveling to different Project HOME sites/sitting/standing.
