Qualifications

• Active Physical Therapy Assistant license or eligibility to practice in the employed state.
• Experience in assisted living, independent living, and memory care settings.
• Proficiency with electronic documentation.
• Strong computer skills with Outlook, Word and Excel.

Responsibilities

• Provide comprehensive therapy services in Assisted Living, Independent Living and memory care.
• Collaborate with rehab therapists and nursing staff in a supportive team environment.
• Maintain timely and accurate documentation.
• Demonstrate flexibility with hours.
