Job summary

Quadrant Health Group in Costa Mesa, CA seeks a Client Care Coordinator with a nursing focus to join our full-time float team. You will conduct nursing assessments at admission, monitor health status, and support medication management across multiple Orange County sites.

Qualifications

LVN license is required for the role.
SUDRC or RADTI certification is required (can be obtained after hire).
Minimum 6 months of detox, behavioral health, or residential treatment experience preferred.
CPR/BLS certification required or obtained before hire.

Responsibilities

Conduct nursing assessments upon client admission and complete intake consents.
Perform ongoing health assessments, including vital signs and behavioral observations.
Monitor and document client health status with accurate reporting.
Conduct COWS and CIWA assessments during detox and treatment.
Oversee self-administration of medication and ensure proper documentation.
Monitor medication compliance and perform counts at least three times per week.
Maintain Centrally Stored Medication Logs and EMR documentation.
Transcribe physician orders and communicate with pharmacies/physicians.
Ensure proper medication destruction and disposal as needed.
Collect and supervise urinalysis (U/A) and BAC samples and ensure accurate reporting.
Perform routine TB tests and audit nursing documentation.
Provide detailed shift reports for continuity of care.
Maintain clean and organized medical stations and supplies.
Assist with pharmacy, medical store, and facility supply runs.
