Qualifications

• Ability to work independently and in a team environment.

Responsibilities

• Provide support with daily activities such as personal care, medication reminders, and meal preparation.
• Assist clients in achieving personal goals, independence, and community participation.
• Support mobility needs, including transfers and lifting.
• Follow care plans to ensure client safety and well-being.
• Document progress and communicate with team members and families.
• Float between clients and shifts based on staffing needs.
