Job summary
Gardner Heights Health Care Center in Shelton, CT is seeking a compassionate Float Social Worker to support residents' social and emotional needs across three Shelton communities.

Qualifications
- Bachelor's or Master's Degree in Social Work or related field.
- Previous experience in a long-term care facility.
- Knowledge of admissions and discharge procedures and residents' rights.
Responsibilities
- Implement and document plans of care related to residents' social/emotional needs.
- Collaborate with nursing, MDS, and rehabilitation to facilitate discharge plans.
- Inform residents, families and staff about residents' rights and health services.
- Maintain resources for residents and families regarding community services.
- Investigate concerns and ensure follow-up and documentation.
- Provide timely referrals to appropriate agencies.
