Responsibilities

As the RN Case Manager, you will assume full nursing responsibility for delivering the Plan of Care for each hospice patient, continuously evaluating your performance and making necessary adjustments to improve care quality.

• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identify and reassess the patient's and family's physical, psychosocial, and environmental needs at least every 15 days.
• Initiate and coordinate the plan of care, maintaining accurate clinical notes.
• Document problems, goals, interventions, and responses to care.
• Collaborate with the patient, family, physician, and other team members in providing daily care.

Education and Experience

Experience Requirements

• One year of clinical experience.

License Requirements

• Current RN licensure in the state of practice.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, access to a reliable vehicle or public transportation.
