Job summary

Circle of Life Home Care and Soaring Eagles Home Care in Waite Park, MN is seeking a Float Registered Nurse Case Manager to provide in-home nursing coverage across our Central Region and Twin Cities area.

You will travel to clients’ homes to deliver hands-on care, supervise caregivers, and coordinate with an interdisciplinary team to ensure compassionate, high-quality outcomes while following each client’s care plan.

Qualifications

• Valid MN RN license
• Willingness to travel regionally within Minnesota
• Experience or interest in home-based care and case management
• Ability to supervise and support caregivers

Responsibilities

• Serve as a float nurse providing clinical coverage for staffing needs in Waite Park and surrounding regions.
• Manage and schedule visits, traveling to clients’ homes for 1:1 nursing care.
• Collaborate with an interdisciplinary team to ensure optimal client outcomes.
• Provide clinical supervision to caregivers and ensure adherence to care plans.

Position Duties Include

• Serve as a float nurse supporting our Waite Park/Central Region and Twin Cities/Metro offices by providing clinical coverage for staffing needs, onboarding support, and organizational growth as needed.
• Manage and schedule your visits. You’ll travel around the area to provide 1:1 nursing services to clients in their home, traveling is required.
• Collaborate with an interdisciplinary team to help provide the best care possible to our clients.
• Provide clinical supervision to our caregivers, making sure they are following the care plan and inspiring them to provide excellent care.

Qualifications

Qualified applicants need a validMNRegistered Nurse license and interest in visiting clients in their homes in the Twin Cities and surrounding area (traveling is required for this role). This position is well suited to an individual that enjoys direct patient contact, supporting a dedicated care team doing good in our communities.
