Job summary

Advocate Health is seeking a licensed Physical Therapist to support four inpatient and three acute care facilities in the Charlotte area, including Carolinas Rehabilitation Charlotte, Pineville Rehabilitation, and Atrium Health locations. You will evaluate, treat, and educate patients across adult acute and rehab settings, ensuring quality outcomes and coordinated care.

Responsibilities include age-appropriate care, treatment planning, documentation, and interprofessional collaboration.

Qualifications

• Active Physical Therapy license in the state(s) of employment.
• Current Basic Life Support (BLS) certification for Healthcare Providers.
• Ongoing compliance with state-mandated continuing education requirements.

Responsibilities

• Age-appropriate patient care and development of age-specific plans.
• Evaluate patient records and referrals; conduct musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary assessments.
• Set goals with patients and monitor outcomes; adjust treatment as needed.
• Educate patients and caregivers; provide home programs and discharge planning guidance.
• Communicate with care teams and coordinate schedules across disciplines.
• Document evaluations, progress notes, education, and billing; ensure billing compliance.
