Qualifications

• RN with geriatrics experience across multiple senior care settings.
• Proficient in medication administration and care planning.
• Experience documenting in Electronic Health Records (EHR).
• Strong infection control and clinical triage abilities.
• Ability to collaborate with interdisciplinary teams.

Responsibilities

• Deliver direct nursing care across skilled nursing, rehab, assisted living, and memory care.
• Assess, monitor, and document resident conditions and responses to treatments.
• Administer medications and treatments per provider orders and policies.
• Float between units and campuses, adapting to changing resident acuity and workflows.
• Collaborate with interdisciplinary teams to develop and implement person-centered care plans.
• Educate residents and families on health conditions, treatments, and self-care strategies.
• Supervise and delegate tasks to CNAs and other staff as appropriate.
• Maintain accurate, timely electronic health records and required documentation.
• Participate in quality, safety, and infection prevention initiatives.
