Qualifications

• Completion of an accredited nursing program.

Responsibilities

• Review patient records using Epic to identify plan of care before rounding.
• Document and track rounding findings in Cipher Rounding Tool; provide weekly updates to leader.
• Provide high level patient service by resolving concerns before discharge.
• Advocate for patients and collaborate with inpatient teams for safe discharge.
• Support complex cases requiring higher level management due to acuity or health literacy.
