H2 Health is seeking a Float Physical Therapist (PT) to support outpatient clinics across the Roanoke, Virginia market, including locations within the H2 Health family such as Lucas Therapies and SORVA Rehabilitation & Wellness. This role offers clinical variety, mentorship, and the chance to impact multiple communities.

You'll evaluate patients with orthopedic, neurological, vestibular, and mobility needs, develop plans of care, and provide evidence-based interventions.

Qualifications

• DPT from a CAPTE-accredited program or equivalent.
• Virginia PT license or eligibility.
• CPR certification or willingness to obtain.
• Ability to travel between assigned clinic locations.
• New graduates are encouraged to apply.

Responsibilities

• Perform comprehensive patient evaluations and develop individualized plans of care.
• Deliver evidence-based treatment interventions to improve mobility, strength, balance, function and quality of life.
• Treat a variety of orthopedic, post-operative, neurological, vestibular and musculoskeletal conditions.
• Provide patient care across multiple outpatient clinic locations in the Roanoke region.
• Collaborate with patients, families, physicians and the interdisciplinary team to achieve outcomes.
• Educate patients on injury prevention, wellness and home exercise programs.
• Maintain accurate documentation of progress and outcomes.
• Utilize Scribe AI to streamline documentation and maximize patient care time.
• Utilize Raintree EMR for documentation, scheduling and patient management.
• Manage a caseload of ~10-12 visits per day for full-time clinicians.

H2 Health is seeking a Float Physical Therapist to support outpatient clinics throughout the Roanoke, Virginia market, including locations within the H2 Health family such as H2 Health, Lucas Therapies, and SORVA Rehabilitation & Wellness. As a Float Physical Therapist, you'll have the opportunity to work alongside multiple experienced clinical teams while treating a diverse outpatient population. You'll provide individualized, evidence-based care to patients recovering from orthopedic injuries, surgery, balance and vestibular disorders, neurological conditions, musculoskeletal impairments, and mobility limitations. Your Role:

• Perform comprehensive patient evaluations and develop individualized plans of care.
• Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life.
• Treat a variety of orthopedic, post-operative, neurological, vestibular, balance, musculoskeletal, and mobility-related conditions.
• Provide exceptional patient care across multiple outpatient clinic locations throughout the Roanoke region.
• Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes.
• Educate patients on injury prevention, wellness, and home exercise programs.
• Maintain accurate and timely documentation of patient progress and treatment outcomes.
• Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
• Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
• Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
• Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.

• Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program.
• Current Virginia Physical Therapist license or eligibility for licensure.
• CPR certification or willingness to obtain as required.
• Ability and willingness to travel between assigned clinic locations.
• New graduates are encouraged to apply.
