H2 Health is seeking a Float Physical Therapist (PT) for its outpatient clinics across Low Moor, Covington, and Daleville, VA. You’ll evaluate patients, develop plans of care, and deliver evidence-based interventions to improve mobility and function in diverse populations.

Qualifications

• DPT or MSPT/BPT from CAPTE-accredited program.
• Virginia Physical Therapist license or eligibility to obtain.
• CPR certification or willingness to obtain.
• Ability and willingness to travel between clinic locations.

Responsibilities

• Perform comprehensive patient evaluations and develop individualized plans of care.
• Deliver evidence-based treatment interventions to improve mobility, strength, balance, function and quality of life.
• Treat orthopedic, post-operative, musculoskeletal, balance, vestibular, and mobility-related conditions.
• Provide excellent patient care across multiple outpatient clinic locations.
• Collaborate with patients, families, physicians and interdisciplinary team to achieve optimal outcomes.
• Educate patients on injury prevention, wellness and home exercise programs.
• Maintain accurate documentation of patient progress and treatment outcomes.
• Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
• Use the Raintree EMR platform for documentation, scheduling, and patient management.
• Manage a caseload of ~10–12 visits per day for full-time outpatient clinicians.
• Contribute to a positive clinic culture focused on patient-centered care.

As a Float Physical Therapist, you'll have the opportunity to work with diverse outpatient populations across multiple clinic locations while providing individualized, evidence-based care. You'll treat patients recovering from orthopedic injuries, surgeries, balance and mobility impairments, musculoskeletal conditions, and other functional limitations. • Maintain accurate and timely documentation of patient progress and treatment outcomes.
• Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
• Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
• Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
• Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.

• Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program.
• Current Virginia Physical Therapist license or eligibility for licensure.
• CPR certification or willingness to obtain as required.
• Ability and willingness to travel between assigned clinic locations.
• New graduates are encouraged to apply.
