Job summary

Northwestern Medicine seeks a dedicated Physical Therapist to provide direct patient care across multiple Chicago-area clinics, covering Oakbrook, Old Irving Park, Evanston, Lakeview, and more. You will document progress, educate patients, and contribute to department improvement.

The role requires a licensed PT in Illinois, a Bachelor’s degree (with a preferred Master’s/Doctorate), and BLS certification. Sign-on bonus and float differential are offered.

Qualifications

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in State of Illinois.
• BLS from American Heart Association upon hire.

Responsibilities

• Assumes complete responsibility over PT patient management from initial contact to discharge.
• Collaborates with treating PT/PTA to maintain patient plans of care.
• Maintains documentation in EMR including evaluations, progress notes and discharge summaries.
• Provides patient education and coordinates care with healthcare teams.
• Participates in committees and department process improvement.
