Job summary

Primary Health Solutions is actively recruiting three Staff Pharmacist positions to join our Provider Team. The staff pharmacist will work within an integrated, team-based care model to improve patient outcomes.

The team includes medical doctors, nurse practitioners, physician assistants, clinical pharmacists, dentists, hygienists, and licensed behavioral health clinicians. A sign-on bonus, generous PTO and benefits are offered to eligible candidates.

Qualifications

• Pharmacist degree or equivalent with board certification.
• Active state license in Tennessee and ongoing CE compliance.
• Experience with patient management programs highly preferred.

Responsibilities

• Review and clarify prescription orders with physicians.
• Dispense medications accurately and verify drug, dose, route and patient.
• Provide patient-specific medication counseling and education materials.
