Job summary

TeleMate Health seeks a Float Patient Support Specialist to provide flexible support across patient care operations and ensure patients receive needed resources and education. The role collaborates with clinical teams to identify barriers to care and assist with discharge planning, adapting to different responsibilities as needed.

The ideal candidate will work remotely, across care coordination duties, and support continuity of care and successful recovery at home, while maintaining HIPAA

Qualifications

• Current, active professional license or credential in one of the following areas: RN/LPN/SW/MA.
• Experience with discharge planning, transitions of care, or care coordination preferred.
• Experience identifying and addressing community resources and social needs.
• Clinical experience in a healthcare setting preferred.
• Fluency in English; additional languages a plus.

Float Patient Support Specialist

We are seeking a compassionate, adaptable, and patient-centered Float Patient Support Specialist to join our dynamic care team. This role is ideal for a Medical Assistant, Nurse, or Social Work professional with experience supporting patients through transitions of care, discharge planning, and community resource coordination.

Position Overview

As a Float Patient Support Specialist, you will provide flexible support across TeleMate Health’s patient care operations, helping ensure patients receive the resources, and education.

This position will work closely with clinical and care coordination teams to identify barriers to care, assist with discharge planning, connect patients with appropriate community resources, and provide follow-up support based on individual patient needs. The ideal candidate will be comfortable working across clinical and social care needs and adapting to different responsibilities as team and patient needs arise.

This is a remote position offering the opportunity to make a meaningful impact on patients’ lives while supporting continuity of care and reducing barriers to successful recovery at home.

Key Responsibilities

• Support patients through transitions of care, including hospital and skilled nursing facility (SNF) discharges and other changes in care settings.
• Assist with discharge planning and identify potential barriers that may impact a patient’s ability to safely and successfully return home.
• Assess patient needs related to transportation, food security, housing, medication access, financial assistance, equipment, home services, and other social or community-based resources.
• Connect patients and caregivers with appropriate community resources, benefit programs, healthcare services, and support organizations.
• Follow up with patients after discharge to identify unmet needs and help facilitate connection to appropriate services.
• Collaborate with nurses, social workers, providers, care coordinators, and other members of the TeleMate Health team to promote continuity of care.
• Communicate with healthcare providers, facilities, agencies, and community organizations as needed to coordinate patient services.
• Provide patient and caregiver education regarding available resources, care plans, healthcare navigation, and next steps following discharge.
• Advocate for patients by identifying barriers and helping ensure their needs and goals are communicated to the appropriate members of the care team.
• Assist with clinical and administrative patient support responsibilities based on departmental and patient needs.
• Review and communicate relevant patient information to support appropriate care coordination and follow-up.
• Accurately document patient interactions, resource referrals, interventions, and follow-up activities in the electronic health record (EHR).
• Monitor outstanding patient needs and follow up to help ensure resources and services are successfully established.
• Identify and appropriately elevate urgent clinical, psychosocial, safety, or behavioral health concerns to the appropriate clinical team member.
• Maintain flexibility to provide support across different areas of the organization as a float resource based on patient volume and operational needs.
• Maintain patient confidentiality and comply with HIPAA, organizational policies, and applicable healthcare regulations.

Qualifications and Skills

• Current, active professional license or credential in one of the following areas:
  • Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN)
  • Social Worker (BSW/MSW)
  • Medical Assistant or other relevant healthcare credential
• Relevant education and/or training appropriate to the professional discipline.
• Experience with discharge planning, transitions of care, or care coordination strongly preferred.
• Experience identifying and addressing community resource and social needs strongly preferred.
• Clinical experience in a healthcare setting preferred.
• Experience working with Veterans and/or VA healthcare systems is a plus.
• Experience working with mental health patients is a plus.
• Strong understanding of healthcare navigation, community resources, and social determinants of health.
• Ability to recognize barriers to care and identify appropriate resources or next steps.
• Strong communication, organization, critical thinking, and problem-solving skills.
• Compassionate, patient-centered approach with a strong commitment to advocacy.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Comfortable adapting to changing priorities and supporting multiple areas of the organization.
• Experience with EHR systems and telehealth platforms preferred.
• High-speed internet connection and a private, secure workspace at home where patient conversations remain confidential.
• Experience supporting geriatric, chronically ill, medically complex, or underserved populations preferred.
• Fluency in English required; additional languages are a plus.
• Must reside in the U.S. and be legally eligible to work for any employer.
• Must pass a background check.
