Brookstone Estates of Harrisburg in Harrisburg, IL is seeking a Licensed Practical Nurse (LPN) to join our PRN staffing pool. The role includes travel to support multiple communities, with an hourly wage of $38.00–$42.00 depending on day vs weekend shifts, and a strong emphasis on safety and resident dignity.

The LPN will oversee medication distribution per physician orders, develop personalized service plans, and communicate with physicians, families and staff to ensure well‑being and quality

Qualificações

• Current state licensure as LPN.
• Must hold valid driver's license; travel may be required.
• Current CPR certification.
• Exceptional communication and interpersonal skills.
• Serve with love, compassion and dignity.

Responsabilidades

• Demonstrate schedule flexibility and travel to support staffing across Communities.
• Oversees medication distribution per physician orders and adheres to medication management procedures for resident safety.
• Create personalized service plans after assessment to support resident independence and dignity.
• Foster clear communication among physicians, healthcare providers and family regarding residents’ wellbeing (physical, mental, social).
• Contribute to quality assurance and performance improvement initiatives.
• Participate in ongoing learning through seminars, staff meetings and training.

Job Details: Job Location: Brookstone Estates of Harrisburg - Harrisburg, IL 62946, Position Type: PRN, Salary Range: $38.00 - $38.00 Hourly, Wage varies: Monday-Friday $38.00 per hour, Saturday-Sunday $42.00 per hour

Responsibilities

• Demonstrate flexibility with schedule and travel to support staffing needs across multiple Communities
• Oversees medication distribution as stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety
• Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity
• Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of communication pertaining to the physical, mental, and social well-being of our residents
• Contributes toward quality assurance and performance improvement initiatives
• Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experience
