Affinity Home Health Care Inc. seeks a Primary Care Assistant – LPN (Float Pool) to deliver direct patient-focused support across multiple clinic settings and assist clinicians during exams and procedures.

You will take vital signs, administer medications as prescribed, support wound care, update EHRs, and help with care coordination and follow-up. Strong communication and ability to thrive in dynamic, collaborative clinics are essential.

Formación

• Current LPN licensure.

Responsabilidades

• Perform and support wound care and related clinical tasks
• Assist providers with exams, procedures, and patient preparation
• Update and manage electronic health record (EHR) documentation
• Support care coordination and follow-up activities
• Educate patients on treatment plans, medications, and self-care instructions
• Adapt to varying practice needs; contribute to quality improvement and ongoing learning
