Licensed Practical Nurse/LPN, Pool , Milwaukee, WI (Sign-On Bonus Available)

Community Care is hiring Licensed Practical Nurses (LPNs) who are committed to teamwork and providing excellent care. Join our compassionate caregiving team and enjoy rewarding opportunities in peaceful, tidy, and well-supplied residential and clinic settings. Pool LPNs may also cover home care.

We offer assignment opportunities on all shifts across our community-based settings, primarily in Milwaukee County, with occasional placements in Racine and Waukesha counties. Our greatest coverage needs are on 2nd and 3rd shifts, as well as weekends.

The Licensed Practical Nurse/LPN provides nursing care and services for our residents. This is a pool opportunity covering all three shifts to provide coverage across all Community Care sites.

Responsibilities include:

• Collaborating with other nursing staff to coordinate and direct care during weekday operations.
• Serving as a care coordinator for weekend and holiday shifts.
• Ensuring care delivery complies with regulations, policies, procedures, and standards established by Community Care, the Program for All-Inclusive Care for the Elderly, and the State of Wisconsin.
• Traveling to various sites as needed.
• Conducting home visits as required.
• Completing the LPN competency checklist within the specified timeline.
• Completing all orientation and onboarding requirements on time.
• Participating in online and in-person competency assessments.
• Attending mandatory trainings.

Qualifications:

• Diploma from an accredited practical nursing program.
• Current Wisconsin LPN license in good standing.
• Minimum of one year experience working with older adults or individuals with developmental and/or physical disabilities in hospital, homecare, or nursing home settings. Practical nursing experience preferred.

Schedule Options:

• Tier 1: At least 2 shifts per month, including one weekend day; if no weekend shifts are available, then 2 weekday shifts are required. One major holiday per year.
• Tier 2: At least 4 shifts per month, including two weekend days; if no weekend shifts are available, then 4 weekday shifts are required. Two holidays per year (one major, one minor).
