Job summary

MyPath is seeking staff to provide direct care and supervision to individuals with special needs across Wisconsin and Indiana, ensuring health, safety, and well-being. You will support daily living activities, meals, mobility, and community engagement while documenting progress and incidents.

Transportation may be required to appointments and outings. The role emphasizes teamwork, adherence to care plans, and a commitment to belonging and inclusive practices within a diverse workforce.

Implement Individual Service or Treatment Plans (ISPs/ITPs), Behavior Support Plans (BSPs), and program routines. Assist individuals with activities of daily living including personal care, meals, mobility, and household tasks. Facilitate activities that promote skill development, independence, recreation, and community engagement. Monitor and document progress, behaviors, health concerns, and incidents in accordance with policy. Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care. Safely transport individuals to appointments, activities, or community outings as required. Maintain a clean, safe, and supportive environment while following all safety and regulatory requirements.
