Job summary

Addus HomeCare is seeking a Licensed Practical Nurse to provide nursing support to clients in their homes across the agency. The role involves assessments, routine nursing tasks under RN supervision, and maintaining compliance with physician orders.

This position offers a flexible schedule with no weekends and no on-call, Monday–Friday self-scheduled hours, and opportunities for growth as nursing services expand. Daily pay and mileage reimbursement are highlighted benefits.

Qualifications

• Graduation from an accredited nursing program and active LPN license in state
• Ability to perform home nursing visits and patient assessments
• Excellent written and verbal communication

Responsibilities

• Medication set-ups and reviews
• Skin assessments and general health evaluations
• Client education and documentation
• Enable patients to stay independent while they remain in their own homes
• Perform routine supervisory visits
• Follow up for unskilled services
• Maintain confidentiality due to sensitive information
• Maintain regular, predictable attendance and flexibility to meet agency needs
• Follow Medicaid and HIPAA regulations and requirements
• Abide by all regulations, policies, procedures and standards
• Performs other duties as assigned
