Job summary

Qualifications

• Initiates, develops, implements, and revises the plan of care with the physician and care team.
• Supervises home health aides and LPNs; provides instruction and assigns tasks per regulations.
• Provides required supervisory visits and coordinates care across disciplines.
• Delivers high-quality clinical services and ensures timely documentation and care coordination.
