Job summary

Frontline Personnel Services is seeking a Social Worker to assess, counsel, interview, and educate clients and families in a skilled nursing setting. This full-time role emphasizes compassionate care and collaboration within the facility.

You will plan and implement support packages, develop treatment plans, and assist with Medicaid/Medicare applications, housing and food assistance as needed. Daily pay options are available.

Qualifications

• Nursing home/skilled nursing facility experience required.

Responsibilities

• Assess patients' situations by interviewing and reviewing history.
• Plan and coordinate support to help patients overcome dependencies.
• Develop and implement individual treatment plans; assist Medicaid/Medicare applications.
• Assist with Food Stamps, Housing applications and phone help.
• Interview clients; assess needs, strengths and weaknesses.
• Set tailored, measurable goals for each case.
• Monitor progress; adjust plans as needed.
• Provide information and counseling during sessions.
• Facilitate referrals to other health professionals.
• Maintain accurate patient documentation
• Promote privacy, dignity and well-being in a safe environment
• Support teamwork and collaboration
• Comply with the Code of Conduct
