Job summary

A healthcare services provider in Brigham City, Utah seeks Independently Licensed Therapists to join their Skilled Nursing Facility Therapy team. This role allows flexibility in scheduling and caseload size. Responsibilities include conducting evaluations and treatment planning both in-person and remotely. Ideal candidates will have a master's degree in a relevant field, Utah licensure, and other qualifications. Competitive pay ranges from $29.00 to $36.00 per billable hour, with options for W2 or independent contracting. Benefits include a 401k match and comprehensive health coverage.

Qualifications

• Must be licensed in Utah as an LCSW, LMFT, or CMHC.
• NPI (National Provider Identifier) required.
• Professional Liability Insurance is mandatory.
• Credentialing with Medicaid/Medicare is a plus.
• Up-to-date vaccinations or appropriate exemptions may be required.
• TB Test may be required.
• CPR/AED/First Aid Certification is necessary.

Responsibilities

• Conduct diagnostic evaluations and treatment planning.
• Complete documentation in a timely manner using EMR (Athena I).
• Comply with agency policies and procedures.
