Job summary

Collage Nursing and Homecare Partners seeks licensed practical nurses to provide daily skilled visits in clients' homes in Dallas, Texas. Flexible shifts are available to fit your schedule, with pay of $80 per visit plus mileage after 30 miles.

Qualifications

• 1 year of LPN home health experience required.
• Current LPN license in state of practice.
• CPR certification current and valid.
• Proof of TB test within required timeframe.
• Ability to work flexible schedules and travel locally.

Responsibilities

• Participate in planning and coordination of total patient care with RN and physician's plan.
• Report and record patient condition and care, noting changes.
• Prepare clinical/progress notes and summaries.
• Assist with activities of daily living and teach self-care.
• Provide and maintain a safe patient environment.
• Assist physician and RN in complex nursing situations.
• Follow universal precautions in patient care.
• Follow emergency procedures for incidents or changes in condition.
