Qualifications

• 1 year experience as an LPN in a home care setting.
• Ability to work a flexible schedule and travel locally.
• Excellent interpersonal skills required.
• CPR certification current and TB test negative.

Responsibilities

• Participate in planning and coordination of total patient care with RN and physician plan of care.
• Accurately report changes in patient condition and care needs.
• Prepare clinical/progress notes and summaries.
• Assist patient with activities of daily living and teach self-care techniques.
• Provide and maintain a safe patient environment.
• Assist physician and RN in complex nursing situations.
• Follow universal precautions in all patient care.
• Follow emergency procedures during catastrophic events.

Skills

Excellent interpersonal skills

1 year home care nursing experience

Valid driver’s license

Education

Graduate of a practical nursing program

Current LPN license

CPR certification

TB test negative
