Job summary

As an LPN in home care, you will plan and coordinate patient care with RNs and physicians, document changes, and teach patients self-care while maintaining a safe environment and adhering to universal precautions.
Qualifications
- LPN with at least 1 year of home care experience.
- Current state LPN license and CPR certification.
- Tracheotomy experience required.
- Graduate of an accredited practical nursing program.
- Ability to work flexible schedules and travel locally.
- Excellent interpersonal skills and reliable transportation.
Responsibilities
- Plan and coordinate total patient care with RN and physician's care plan.
- Report changes in patient's condition and care, including signs and symptoms.
- Prepare clinical/progress notes and summaries.
- Assist patients with activities of daily living and teach self-care.
- Provide and maintain a safe environment for the patient.
- Assist physician and RN in complex nursing situations.
- Follow universal precautions in all patient care.
- Follow emergency procedures for catastrophic events.
