Qualifications

• Active Maryland RN license.
• Minimum of one year of recent clinical (medical/surgical) experience.

Responsibilities

• Follow a designated care plan in accordance with patient needs.
• Make home visits to clients in designated geographic territories in Maryland.
• Administer medications, wound care, treatments, and procedures as prescribed.
• Monitor client conditions and report changes to Clinical or Client Services Manager.
• Document observations, interventions, and evaluations in EMR.
• Coordinate case management and care with the team.
