Qualifications

• Graduate of CAPTE-approved PT program.
• Current PT licensure in state and CPR (AHA) certification required.
• Two years of adult PT experience; home health preferred.

Responsibilities

• Perform physical therapy evaluation and treatment per physician orders, including adaptive equipment recommendations.
• Set measurable goals and develop plans to maximize independence and function.
• Maintain communication among the treatment team, patient, and family.
• Document services thoroughly and timely for proper billing.
• Supervise PT aides and assistants in line with state practice act.
