Job summary

AW Health Care is seeking Registered Physical Therapists to provide skilled, 1:1 physical therapy in patients’ homes with a focus on work‑life balance. This part-time role offers flexible scheduling and a supportive team in the St. Louis area, serving Missouri and Illinois for over 25 years.

You will work under supervision, perform home visits, and contribute to patient outcomes while managing documentation efficiently.

Qualifications

• Graduate from a physical therapy curriculum approved by APTA/CAAHEP/Council of Medical Education and APTA. Currently licensed in the practicing state(s).
• Two years' experience preferred.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Provides skilled, intermittent physical therapy in patients’ homes according to physician orders and care plans.
• Works under supervision of Clinical Directors/ Rehab Director.
• Writes reports to physicians regarding patient progress.
• Instructs patients and families in home programs and activities of daily living.
• Completes and submits OASIS assessments, reassessments, transfers, and discharges.
