Job summary

CWI in New York is seeking dedicated Community Support Professionals to provide Community Habilitation and Respite services across Warren, Washington and Northern Saratoga Counties. Community Habilitation helps individuals with intellectual and developmental disabilities build independence through everyday activities, while Respite offers caregivers a much needed break.

Qualifications

• High school diploma preferred.
• Pass a standard background check.
• Valid driver's license with acceptable driving record.
• Complete First Aid/CPR and safety training.
• Minimum of one year experience with people with disabilities.

Responsibilities

• Work independently, with supervision and support from the Community Services Team.
• Provide person-centered services aligned with individualized Staff Action Plans, reflecting each individual's goals, strengths, and needs.
• Support, supervise, and guide individuals as they pursue their chosen goals.
• Use the electronic health records system to document services.
• Attend occasional meetings and in-service training as required.
• Interact with individuals in a therapeutic manner to promote social and interpersonal development.
• Model positive, professional behavior in interactions with community members and other professionals.
• Deliver flexible, adaptive, and appropriate care and supervision to the individuals they support including assistance with daily living activities, community outings and social engagements.
