Responsibilities

• Follow designated care plans to patient needs.
• Perform home visits in geographic territories.
• Administer medications, perform wound care and procedures.
• Monitor patient conditions and report changes to supervisor.
• Document doctors' orders and patient observations accurately.
• Conduct client assessments and coordinate case management.
• Utilize EMR/tablet to document care activities.
